Essay

The object of the research is kidney lesion in systemic vasculitides (SV); the aim is to improve the quality of the diagnostics and prognosis of kidney lesion course in the patients suffering from SV, to reveal new links in the chain of the pathogenesis of the illnesses; the methods employed are clinical, instrumental, biochemical, immunological, immunoenzyme, immunoblot, biophysical, physicochemical, morphological, hygienic and statistic ones; the novelty and the results are character caused by Takayasu's ajrtjarteritis, Schonlein-Henoch purpura, cryoglobulinemic vasculitis, microscopic polyangiitis, Wegener's granulematosis with polyangiitis and polyarteritis nodosa based on clinical, laboratory, sonographic and morphological data, revealing the interconnection between some particular manifestations of nephropathy as well as the rate which it progresses and the extra-renal disease components, the research is the first to reveal the clinicopathologic connection between the blood levels of antineutropholic cytoplasmatic antibodies and other immunity factors (levels of fibrinogen, immunoglobulin A, G, M, circulating immune complexes, rheumatoid factor, DNA and cardiolioin antibodies), to determine the impact of the structural signs of glomerules, tubules, stroma and vessels lesion based on the results of light and immunofluorescence microscopy of renal biopsy samples as well as biochemical and physicochemical parameters of urine (levels of proteinuria, fibronectinuria, 2-microglobulinuria, creatininuria, ureauria, uricuria, nitrituria, values of superficial tension and relaxation, viscoelasticity module), to prove the interconnection between the endothelial function of the vessels and adsorption-rheological properties of the blood on the patients with SV, the importance of the level in the blood of patients of endothelin-1, thromboxane-A2, homocysteine, prostacyclin, nitrites, cyclic guanosine monophosphate and the levels of volume viscosity of blood, superficial viscosity, elasticity, tension, relaxation, viscoelacity module in clinical-pathogenetic structures of the illnesses, including nephropathy, to prove the dependency of progress and course peculiarities of kidney lesion in patients with SV on the ecological factors of breathable air pollution with ammonia, benzopyrene, hydrogen sulphid, phenol, dioxides, and drinking water - with toxic and essential microelements, nitrates, sulfates, phosphates and chlorides; based on clinical, laboratory, sonographic and morphological research methods prognostic factors of SV course as a whole, nephropathy in particular, are established, criteria are developed that allow evaluating the rate at which kidney pathology progresses and improving the quality of diagnosing kidney lesion and some particular features of kidney damage in the patients with SV; implemented in the activities of the CI "Dnipropetrovsk Municipal Clinical Hospital № 2", NSC "M.D. Strazhesko Institute of Cardiology" MAS of Ukraine, rheumatology units of the CE "Zaporizhia Regional Clinical Hospital", Clinical Hospital "Feofaniya" SAA (Kyiv), Ivano-Frankivsk Regional Clinical Hospital, A. Novak Transcarpathian Regional Clinical Hospital, Lviv Regional Clinical Hospital, Pidvolochynsk Communal Central District Hospital, SSE "Scientific and Practical Center for Preventive and Clinical Medicine" SAA, National Specialized Children's Hospital, as well as in the pedagogical activities of Donetsk National Medical University of the Ministry of Health of Ukraine, SE "Dnipropetrovsk Medical Academy of the Ministry of Health of Ukraine", SHEE "Ivano-Frankivsk National Medical University of the Ministry of Health of Ukraine", SHEE " I.Ya.
