Rybert Y. A comprehensive approach to diagnosis and treatment of patients with both temporomandibular disorders and disturbances of functional occlusion

The thesis presents a theoretical generalization and new solution of research and practice problem of medicine, in particular dentistry which results in efficiency improvement of diagnostics and treatment of patients with temporomandibular disorder with impaired functional occlusion on the basis of diagnostic and treatment processes training intended to reinitiation and normalization of occlusal ratio and functions of TMJ.Based on clinical research, a comprehensive regimen of diagnosis of temporomandibular disorders was developed, which enabled to carry out their qualitative and quantitative assessment.In addition to the clinical and radiological examinations, the proposed algorithm includes the use of electronic axiography and ultrasound examination of the TMJ condition ensuring differential diagnosis of disorders of functional occlusion and function of temporomandibular joint. The algorithm for treating patients with temporomandibular disorders and impaired functional occlusion was improved. It includes: therapeutic and surgical training; occlusal therapy with the use of various types of occlusal splints; orthodontic treatment; correction and balancing of functional occlusion by selective grinding of teeth and, if necessary, direct restoration of teeth with composite materials using silicone templates; stabilization of occlusion by temporary prosthetic restorations; orthopedic treatment by reconstruction.
