The dissertation presents theoretical substantiation and practical solution of the actual problem - increasing the effectiveness of treatment of generalized periodontitis in patients with endothelial dysfunction in cardiovascular pathology. The research was aimed at determining the prevalence, peculiarities of the course of generalized diseases of periodontal tissues associated with cardiovascular pathology, and deepening the understanding of the pathogenesis of these comorbid states on the basis of the study of immune and molecular mechanisms of the development of pathological changes in the periodontal and endothelial tissues in order to create and justify a new differentiated approach to the comprehensive treatment of periodontium inflammatory diseases in patients with this category. The result of the work was a new concept of the pathogenesis of generalized periodontitis in cardiovascular pathology and the development and substantiation of new protocols for the treatment of this disease based on the combination of primary periodontal treatment (Phase I) using modified PPR therapy and systemic treatment, in accordance with existing protocols for the elimination of endothelial dysfunction and an additional use of antioxidant, a donor of nitric oxide, which allowed to obtain a stabile therapeutic effect for 3-6 months which was confirmed by significant (P <0.05) reduction of the objective indicators of periodontal and hygienic indices electron microscopic data and biochemical studies.
