On the basis of a integrated study of clinical, instrumental, biochemical and immunological evidence patients with type 1 and 2 diabetes, hypertension, obesity and polycystic ovarian syndrome, are presented a generalization and proposed mechanisms for the development of endothelial dysfunction in insulin resistance states, new approaches to early diagnosis of disorders endothelial function were developed, taking into account the role and prognostic significance of endothelial vasoactive factors, of the disruption of carbohydrate, lipid metabolism, insulin sensitivity, obesity in its development. The method of determination and evaluation of endothelium dependent dilatation based on traditional risk factors of cardiovascular pathology and endothelial vasoactive factors was proposed. The prognostic criteria for the development of arterial hypertension in type 1 and 2 diabetes were established. For the first time the computer program "Estimation of probability of development of arterial hypertension in diabetes" was developed and implemented in practical activity. The experimental method of correction of oxidative and nitrosative stress, cardiac and endothelial dysfunction was investigated at genetic induced arterial hypertension and insulin resistance, experimental diabetes.
