The dissertation presents the results of the microbiological, clinical substantiation of a new strategy of the use of decamethoxin antiseptics for the prevention of complications of microbial etiology in patients with burns. In microbiological examination of exudates, received from wounds of 581 patients with burns of 2nd-a-b-3rd degrees (10-85% of the body surface), qualitative and quantitative composition of microorganisms and their biological properties were studied. Polyantibiotic resistance, prognostic dependencies of indicators of decreased sensitivity to beta-lactams, aminoglycosides, fluoroquinolones of the dominant pathogens S.aureus, A.baumannii, P.aeruginosa were established. The VIM-determined resistance of carbapenems to P.aeruginosa (11.81%), A.baumannii (2.7%), P.mirabilis (17.65%) was proved. The results of a comprehensive study of the antimicrobial properties of antiseptics, new antimicrobial compositions, their effect on antibiotic sensitivity and biofilm-forming ability of certain types of opportunistic microorganisms have been presented. The effect of decamethoxin agents on S.aureus, A.baumannii was studied by means of electron microscopy. The absence of cytotoxic, pro-apoptotic effect of decamethoxin agents has been proven. There were clinically substantiated the advantages of the use of decamethoxin antimicrobials with prolonged action in the prevention of microbial complications in patients with burns.
