Essay

As a result of retrospective analysis of 450 case histories of dental inpatients and 500 outpatient dental cards for the period from 2016 to 2019, typical approaches to anesthesiological support of planned surgical interventions in the maxillofacial area, which are empirical. Significant negative "pain" experience in 21,6% of patients during previous surgical dental treatment is the cause of dental phobias, which indicates the need to introduce into clinical practice new methods of preoperative anti-stress preparation of patients, improving intraoperative local anesthesia, postoperative treatment of pain syndrome, individual use of ataractics, non-narcotic analgesics and their adjuvants. In the postoperative period of the treatment of pain syndrome and acute inflammatory reaction in the maxillofacial area of the main group, patients were prescribed individualized schemes of taking of lornoxicam and dalargin, compared their effectiveness with standard approaches to the treatment of pain syndrome. It was found that in patients of the main group with a high level of neuroticism on the background of intravenous xefokam and dalargin there was a rapid increase in of β - endorphins and leu – enkephalins in blood. During their maximum growth (from the 1st to the 3rd day of the postoperative period), it was revealed differences in the concentration of β - endorphins - p = 0,421615 (in the main group – 19,65 ± 2.41 pg / ml and 16,93 ± 2,31 pg / ml - in the comparison group) and leu - enkephalins - p = 0,039171 (in the main group – 186,51 ± 7,94 pg / ml and 162,43 ± 7,85 pg / ml - in the comparison group). The obtained results of the scientific research indicate the advantages of the proposed approach to anesthesiological support of planned surgical interventions in dental patients over standard ones.
