Essay

The thesis studies the condition of the temporo-mandibular joints, complex diagnostics, clinical pattern, prosthetic treatment and prevention of the muscular-joint dysfunction of the temporo-mandibular joints. The study served a basis for searching for, clinical introduction and assessment of the algorithm of complex diagnostic, treatment and prevention of the muscle-joint dysfunction of the temporo-mandibular joint. A total of 1073 patients of the O.Bogomolets National medical university Stomatological medical center and O.Bogomolets National medical university department of prosthetic stomatology were examined, from which 274 patients were diagnosed and treated according to the offered algorithm. Using the retrospective analysis of previous years (2009-2019), the dynamic pattern of the temporo-mandibular dysfunctions was studied, the complications and recurrences analyzed. The obtained data served a basis for the search and creation of new methods for algorithms of diagnostics and treatment of the pathology. The paper presents theoretical generalization and the scientifically based practical solution of the urgent prosthetic stomatological issue – improved diagnostics, prosthetic treatment and prevention for the patients with the muscular-joint dysfunction of the temporo-mandibular joint. Using the multi-disciplinary approach and modern computer technologies, a complex model of the disease pathogenesis was founded. The author scientifically bases a concept of the muscle-joint temporo-mandibular dysfunction pathogenesis, which joins all existing theories: the articulation-occlusion, neuro-muscular, psychophysiological, psychosomatic and the ischemic one. The concept served a basis for developing the applied software – an application. The approach provides for comparing and separating the dominant chain of dysfunction in the temporo-mandibular joint. Due to all conducted studies, in order to synchronize the data, a complex applied computer application software for the TMJ dysfunction diagnostics “PR” was introduced. This resulted in a new complex approach using the multi-mode computer applied software application “PR”. The diagnostic data stipulated for introduction of the clinical diagnostics and treatment algorithms. The author compared and analyzed the results and effectiveness of treatment of the patents with the temporo-mandibular joint dysfunction, using the multi-mode complex of diagnostics and treatment of the pathology, proving its effectiveness. This substantial approach to the presented in the paper model of diagnostics and treatment of the patients with the temporo-mandibular joint dysfunction, and, according to this, presented synchronized complex of the pathology studied substantiated for evaluation of the qualitative and quantitative changes in the biosystem effector chain of masticatory apparatus, providing for the basic temporo-mandibular joint system functions.
