Essay

The dissertation presents the results of research in the surgical treatment of patients with chronic pancreatitis with pancreatic and biliary hypertension at the stages of fibrous transformation of the pancreas. Based on retro- and prospective analysis, the main differences in the histological structure of the pancreas depending on the duration of the disease and changes in the structure of the pancreatic parenchyma before and after surgical treatment of chronic pancreatitis and pancreatic pseudocysts were established. A clinical and morphological comparison was made at the stages of fibrous transformation of the pancreas in chronic pancreatitis. The peculiarities of the fibrous transformation of the pancreas in chronic pancreatitis, changes in the architecture of the pancreas in CP with pancreatic and biliary hypertension were studied, and their relationship with biochemical indicators of inflammation and the degree of fibrosis was established. Changes in the quality of life before and after surgical treatment were analyzed, and an own questionnaire was developed. The study of the validity of imaging and laboratory non-invasive markers for preoperative assessment of inflammatory activity and the stage of fibrous transformation of the pancreas in chronic pancreatitis in the clinic was conducted, and a differential diagnostic algorithm was created. New treatment tactics and new methods of surgical interventions have been developed and the advantage of their use in chronic pancreatitis and pseudocysts of the pancreas has been proven.
