Object - the patients with ischemic heart disease (IHS) and silent myocardial ischemia. Aim - to work out the diagnostic and prognostic criteria for silent myocardial ischemia, the new differential approaches to the therapy based on determination of endothelial and platelet dysfunction in patients with IHS. Meth-ods: exercise test, Holter ECG monitoring, ultrasound and X-ray investigations, blood lipid detection, evaluation of cloating and fibrinolytic plasma activity, concentrations of prostaglandi-nes, cyclic nucleotides, catecholamines, endopioids; analisis of morphologic and functional platelent properties. On the grounds of the data obtained some diagnostic and prognostic criteria of silent ischemia and the new approaches to its treatment and ef-ficacy control are worked out.
