Essay

The problem of preserving reproductive health in women of childbearing age is of strategic importance for the sustainable development of society and is one of the most urgent in Ukraine. Concerning the preservation of reproductive health, the pathology of the thyroid gland, which accounts for 46.7% of all endocrinopathies, attracts great practical and theoretical interest. Particular attention should be drawn to subclinical hypothyroidism, which provokes the imbalance of the activity of the hypothalamic-pituitary-ovarian system with clinical manifestations of menstrual cycle disorders, organic gynaecological diseases and fertility problems. In addition, structural and functional disorders are developing in all, with no exception, organs and systems of the organism, indicating the formation of a comorbid state. At present, the timely diagnosis and treatment of comorbid conditions by subclinical hypothyroidism in gynaecology is often very difficult because of intricacy to determine the leading link in reproductive health disorders. Uniform tactics for such patients has not been developed and opinions on the use of substitution levothyroxine therapy are ambiguous. There are no approaches to integrated health assessment in conditions of comorbidity and reproductive health violation, as well as control of the effectiveness of medical and preventive measures. The approaches to a simple and accessible algorithm for gynaecologist action in reproductive health disorders in women of reproductive age with subclinical hypothyroidism are not sufficiently developed.
