Essay

The dissertation proposes to improve the therapy of patients with chronic kidney disease (CKD) stage V, treated with hemodialysis (HD) by using the drug melatonin based on the study of melatonin-forming function of the pineal gland (MFE) and its relationship with age, sex, "experience" dialysis therapy, clinical-anamnestic and laboratory parame- ters, parameters of daily blood pressure monitoring, quality of life, severity of sleep disor- ders. The urgency of this goal is due, on the one hand, to the annual steady increase in the number of patients with CKD requiring treatment with renal replacement therapy, on the other hand, the increase in cardiovascular complications and mortality. The development and progression of CKD is associated with adverse changes in all areas of patients' lives and, as a result, leads to significant medical and social costs. Due to the involvement of more advanced renal replacement therapy technologies it is possible to extend the life ex- pectancy of patients with end-stage renal disease. However, the issues of improving the effectiveness of antihypertensive therapy, correction of disorders of purine and lipid me- tabolism, improving the quality of life and sleep in this, especially severe, cohort of pa- tients, which is an urgent task of modern medicine. The purpose and objectives of the study: based on the study of MFE in patients with CKD stage V, who are on HD, and its relationship with age, sex, clinical history and met- abolic parameters, daily blood pressure monitoring parameters, quality of life, severity of sleep disorders, increase efficiency therapy by using melatonin.
