Essay

Object of study: stage II hypertension with overweight. The study objective: treatment efficiency increase in hypertensive (stage II) overweight patients with angiotensin-converting-enzyme inhibitor or angiotensin II receptor blocker in combination with calcium antagonist and statin personalized alternative use on the basis of cytokines and adiponectin influence on the left ventricle (LF) remodeling, central and cerebral haemodynamics studying. Research methods: general clinical; anthropometric; clinic anamnestic; laboratory (biochemical blood tests, which included lipid profile, liver functional state); immune-enzymatic (pro-inflammatory cytokines determination – interleukin-1β (IL-1β), tumor necrosis factor – ά (TNF-ά), anti-inflammatory cytokine – IL-10, adiponectin (ADP) in blood plasma); instrumental (echocardioscopy, the head and neck vessels duplex ultrasound, electrocardiography in 12 leads); statistical (parametric, nonparametric, correlation analysis). Scientific novelty of the obtained results: clinic anamnestic, biochemical parameters, including lipid profile, which are characteristic for stage II hypertension with overweight were analyzed in detail. Features of left ventricular remodeling and extracranial carotid and vertebral arteries changes in hypertensive overweight patients have been identified. For the first time, systemic immune-inflammatory factors and adiponectin changes in comparison with heart and blood vessels remodeling degree in hypertensive (stage II) overweight patients were studied. For the first time the complex therapy with antihypertensive drugs and 3-hydroxy-3-methyl- glutaryl-coenzyme A reductase inhibitors inclusion effectiveness in the systemic immune-inflammatory factors correction, serum adiponectin levels, central and intracardiac hemodynamics and heart and vessels structural functional state and therapeutic intervention proposed program were evaluated.
