Essay

The urgent problem of perinatology worldwide are preterm birth, intensive care and nursing of premature babies. Despite significant advances in neonatology and improvement therapeutic strategies, early-onset neonatal sepsis (EONS) remains leading cause of morbidity and mortality. Premature birth baby born associated with an increased risk of EONS and subsequent the development of the adverse effects of the disease. Lack of specific clinical symptoms and insufficient diagnostic value of existing laboratory tests leads to limited reporting of cases of this diseases and high-level empirical antibacterial prescribing therapy. Therefore, today, in order to reduce the incidence rate, disability and mortality of newborns, relevant scientific the direction is the search for new effective diagnostic markers of EONS with high prognostic significance. To solve the tasks set in the work, was analyzed clinical and laboratory observations of 102 preterm infants. Newborns who have been screened as a part of this dissertation works were divided into groups: first group (n = 42) were prematurely born children with EONS, second group (n = 48) – with intrauterine pneumonia (IP), the control group included 12 premature infants without implementation of perinatal infection. Survey program included analysis of perinatal history data, conducting standard clinical, laboratory and instrumental research according to the current orders of the Ministry of Health of Ukraine, definition of indicators systemic inflammatory response: C-reactive protein (CRP), procalcitonin (PCT) and sTREM-1 content studies blood serum in the first day of life, as well as in the dynamics of early neonatal period. Comparative analysis of the course of the antenatal period of the studied groups found significant differences between the frequency of obstetric pathologies gynecological profile. Dominant position in the structure of amniotic states the cavity belonged to oligohydramnios, which was noted in the anamnesis of 47,6 % children with EONS. Chorioamnionitis and a long anhydrous period were found in 30,9 % and, accordingly, 28,6 % of group first pregnancies.
