On the basis of clinical and instrumental research and statistical analysis of the dissertation Nikolaienko A.S " Optimization Of Diagnostics And Treatment Of Trophic Ulcers Of The Lower Extremities In Petients With Type 2 Diabetes Mellitus " identified scientific and theoretical provisions, which together allowed to theoretically generalize and identify new ways to solve the problem of treatment of patients with trophic ulcers of the lower extremities on the background of type 2 diabetes. . In the course of work the influence of concomitant diseases and some other factors on the development of neuropathic ulcers of the lower extremities in patients with type 2 diabetes has been clarified. The nature of morphological changes in the area of ulcerative defects in patients with type 2 diabetes has been clarified. For the first time, the features of the components of the proteolytic system (on the example of neutrophilic elastase) in patients with type 2 diabetes complicated by ulcers of neuropathic origin have been studied. The activity of markers of angiogenesis and lymphangiogenesis in ulcers of neuropathic genesis in patients with type 2 diabetes has been studied. Possibilities of autologous platelet-rich plasma (aPRP). use in patients with neuropathic ulcers on the background of type 2 diabetes have been studied and its effectiveness on the expression of receptors for endothelial growth factors (VEGF) and CD 31, Podoplanine, which are markers of angiogenesis and lymphogenesis, has been proved. The results of traditional methods of treatment (according to protocols) and the method proposed by the author of the dissertation research in the near and long rehabilitation period are compared.
