Essay

It was found that the coverage of the population with mandatory vaccinations in recent decades was unsystematic and did not provide adequate level for community immunity, especially in 2014-2016 (0-40 % of demand). Described situation directly correlated with the lowest levels of vaccine availability during this period (rxy = 0.47-0.78) and could be a direct cause of epidemic outbreaks and infection cases in the years following low immunoprophylaxis coverage. Vaccination coverage after outbreaks has increased with declining refusal rates. It was revealed that the criteria of contraindications to immunization require legal regulations. Some evidence for that are: significant frequency fluctuations of contraindications to vaccination of children with the DPT vaccine (2.3-11.5 %) during the last decade in Ukraine, significantly lower levels and a downward trend in Ivano-Frankivsk region, a direct correlation with the presence of the vaccine (rxy = 0.46), 15.1 times higher (compared to long-term) frequency of temporary contraindications and their seasonal nature. More evidence is present with direct interdependence of low coverage of children under 7 months of age with a full course of vaccination against pertussis, diphtheria, tetanus (52.5 % in Ukraine and 33.0 % in Prykarpattia in 2020) with the frequency of contraindications to vaccinations (rxy = 0.56). The first to: problematic issues of organization of Ukrainian population vaccination at the present stage of socio-economic development of the country and transformation of the public health system are systematically presented; regional and all-Ukrainian features of dynamics and structure of population morbidity on vaccine-controlled infections indicators are revealed; attitude and coverage of vaccination and revaccination of medical workers as risk groups on an occupational basis, as well as their awareness of the organization and conduct of immunoprophylaxis is studied; scheme of three-level acquisition of communicative skills of counseling of the population on immunoprophylaxis is substantiated and developed; scientifically substantiated, developed and proved the expediency and predictive efficiency of the functional-organizational model of improving population immunization management system which is complex, integrated, formed on the principles of combining organizational and communication strategies stratified for different target audiences. The scientific and methodological approach to the development of healthcare workers and population adherence to vaccination is improved. Knowledge of the connection between immunization coverage, vaccine presence and availability, contraindications and vaccination refusals is further developed.
