Essay

Chronic ischemia of the lower extremities in atherosclerotic lesions is an urgent problem today. At occlusion of a superficial femoral artery of provide blood supply of an extremity is provided on collateral branches of DFA. Affected by atherosclerosis DFA with stenosis more than 50% deepens ischemia of the limb, leads to swelling of the foot and legs. In patients with intact DFA, chronic ischemia of the lower extremities is less pronounced and limb function is preserved for a long time. With all the evidence of the leading and only major role of DFA in the collateral blood supply of the lower extremity in atherosclerotic lesions of the femoral-popliteal-tibial segment, profundoplasty operations are indirect methods of revascularization. Apparently for this reason, profundoplasty as an independent method of treatment of chronic lower extremity ischemia is used less often, and some issues related to architecture, morphometry and diagnosis of DFA, method and type of deep femoral artery plastics, evaluation of long-term treatment outcomes need to be addressed. Profundoplasty is an arterial reconstructive operation that does not restore the patency of the SFA, but restores collateral blood flow to the thigh, improving flow in the knee joint and the microcirculatory tract of the lower third of the leg and foot.
