Essay

The dissertation is devoted to the surgical treatment of ascending thrombophlebitis of the great saphenous vein, Acute ascending thrombophlebitis of the great saphenous vein is a dangerous complication of lower extremity varicose vein disease. Thrombotic masses that are located more proximal or at the level of the sapheno-femoral junction may spread to the common femoral vein. This may result in the development of deep vein thrombosis and thromboembolism. A present, surgical treatment of acute ascending thrombophlebitis of the great saphenous vein consists in resecting the thrombosed great saphenous vein on a probe or from individual cuts. High probability of surgical injury of traditional phlebectomy, the large number of complications in the postoperative period and continuous follow-up care triggered the search for alternative and less traumatic treatment modes. This implies the main purpose of the thesis research – improving the results of surgical treatment of patients with acute ascending thrombophlebitis of the great saphenous vein through the use of the method of endovenous high-frequency electric welding. The obtained results are morphologically substantiated – treatment using high-frequency electric welding on the thrombosed great saphenous vein made it possible to determine the optimal welding parameters, thereby allowing to improve the method of endovenous high-frequency electric welding in the surgical treatment of acute ascending thrombophlebitis of the great saphenous vein. This result can be obtained by connecting the tissues as a result of the passage of high-frequency electric current through them. Welding of a thrombosed great saphenous vein occurs due to the effect of electrothermal denaturation of protein molecules. The introduction of the results of the thesis research, namely the improved method of endovenous high-frequency electric welding in the treatment of acute ascending thrombophlebitis of the great saphenous vein into clinical practice, will improve the functional and cosmetic results. It will also accelerate recovery, reduce the time of treatment and follow-up care of patients with acute ascending thrombophlebitis.
