The dissertation is devoted to the study of the course of chronic hepatitis C in patients with different variants of the TLR-4 gene polymorphism. The effect of different allelic types of the TLR-4 gene on the effectiveness of antiviral therapy in patients with chronic hepatitis C with genotype 1b also was studied. The focus of this study was to determine the main characteristics that estimate the severity of clinical course of chronic hepatitis C and their relationship with the TLR-4 gene polymorphism. The obtained data indicated that the presence of the minor C +3725 G/C allele of the TLR-4 gene in the patient was associated with more pronounced manifestations of all clinical and biochemical syndromes that characterized the severity course of chronic hepatitis C. Individuals, which are carrying the C allele of +3725 G/C TLR-4s gene, have a significantly higher risk of severe liver fibrosis (F3-F4) and high viral load (> 600,000 IU/L), compared with those, who did not have this allele. The chance of achieving a stable virological response during antiviral therapy in patients with chronic hepatitis C with genotype 1b was 13 times higher in patients, who did not have the C +3725 G/C allele of the TLR-4 gene.
