Essay

The dissertation is devoted to increase the effectiveness of diagnosis and treatment of localized scleroderma on the basis of study of endothelial, endotoxic and prooxidant metabolic disorders, their connection with the severity of disease and effectiveness of pharmacotherapy. The role of endothelial disorders, oxidative stress and endotoxicosis in the pathogenesis of localized scleroderma was investigated for the first time. It was established that high concentrations of endothelin-1, vascular endothelial growth factor A (VEGF-A), vascular endothelial adhesion molecule (VCAM) in the blood, hyperactivation of the processes of peroxidation of lipids and proteins against the background of a deficiency of the endogenous antioxidant glutathione and low activity of the enzymatic antioxidant link are integrated in the development of dermatosis. The connection of certain metabolic disorders with the degree of severity of the disease, clinical forms of dermatosis of the area of skin damage, duration of dermatosis, gender, and age of patients was revealed. For the first time, it was found that the use of complex therapy, which included the appointment of L-arginine, an antioxidant drug - thioctic acid, colloidal silicon dioxide, ultraphonophoresis with hyaluronidase, corrected metabolic disorders in the body of patients with localized scleroderma, reduced the levels of endothelial markers, manifestations of endotoxicosis, restored the balance in system of pro- and antioxidants, reduced the intensity of free radical oxidation processes and improved the clinical effectiveness of dermatosis treatment.
