Essay

The work is devoted to the problem of choosing algorithms for the diagnosis and treatment of female children with surgical pathology of the pelvic organs, including the combined pathology of the appendages of the uterus and appendix and dissertation is an experimental, clinical and morphological study based on patentable scientific developments. Some factors of the clinical course of acute surgical pathology of the pelvic organs in girls of the Podilsk ethno-territorial region of the Vinnytsia region were studied, which showed, in the absence of a certain pattern of quantitative distribution, their general annual tendency to increase. The evaluation of morphological and morphometric studies of ovarian biopsies on the experimental model of their torsion in female white rats convincingly proved the possible maximum duration of preservation of the histological integrity of the gonads, due to the restoration of blood circulation in them after detorsion, in the period of pathology up to 72 hours. Reasonable feasibility of use in the diagnosis of acute appendicitis integral index of intoxicationas SIEI – highly sensitive marker for endotoxicosis in the course of acute appendicitis in children. It was determined for the first time that the level of a cytokine ССL2/МСР-1 allows, in each specific case, to predict the course and prevent possible complications of the postoperative period in girls with acute gynecological pathology, considering the pro-inflammatory chemokine indicator as the activity of an immunopathological process with an inflammatory reaction in response to the magnitude of surgical trauma. An algorithm for the diagnosis and complex treatment of severe surgical pathology of the pelvic organs in girls was developed based on a multidisciplinary approach anduse of minimally invasive endosurgical methods of treatment, which allowedin the prospective period.
