Essay

The dissertation is devoted to the problem of choosing the tactics of surgical treatment of papillary thyroid cancer aimed at reducing the incidence of iatrogenic complications and improving the quality of life, while maintaining the principles of oncological surgery and minimizing the risk of recurrence. The aim of the study is to improve the quality of life of patients with pT1a- bN0M0 papillary thyroid cancer by minimizing the volume of surgery without increasing the risk of recurrence. To determine the incidence of recurrence after surgical treatment of papillary thyroid cancer in patients with pT1-2N0M0, a retrospective study analyzed the results of treatment of 562 patients from 2015 to 2020. 178 patients underwent organ-preserving surgery in the form of hemithyroidectomy with central neck lymphadenectomy, 384 patients underwent total thyroidectomy with central lymphadenectomy. During surgical treatment, an obligatory step was an intraoperative rapid histological examination of the removed lymph nodes and primary tumor to confirm the spread (absence of metastasis (iN0) and extra- organic invasion of the primary tumor (iT1-2).
