The work is devoted to the study of the specifics of the action of antiepileptic in the treatment of seizures in mice on the model of corneal kindling after prior application in low doses of an inhibitor (sultiam) and an inducer (carbamazepine) of enzymes of the cytochrome P450 system, and a comparison of the obtained results with an analysis of clinical and pharmacogenetic features of treatment-resistant epilepsy in children. The study of drug-resistant epilepsy in strictly determined laboratory conditions made it possible to reproduce and understand some mechanisms related to the selection of therapy, replacement and combination of drugs, and especially in situations where patients have certain genetic features of metabolism. Conducting an experimental study made it possible to confirm the value of modulation of enzymes of the cytochrome P450 system for the effectiveness of treatment of pharmacoresistant epilepsy. We developed recommendations for the treatment of drug-resistant epilepsy. Choosing antiepileptic drugs, changing treatment, using polytherapy, it is necessary to take into account not only the etiology, nature of epileptic attacks, age, gender, but also the interaction of drugs, their effect on the activity of enzymes of the cytochrome P450 system.
