The aim of this study: based on the study of clinical-psychopathological, pathopsychological features and assessment of the quality of life of patients with anxiety-depressive disorders with eating disorders, to develop a comprehensive, phased, differentiated treatment and rehabilitation program for this category of people and evaluate its effectiveness.
Clinical-psychopathological and pathopsychological features of anxiety-depressive disorders with eating disorders were studied in the dissertation. The relationship between eating disorders, body image, and anxiety-depressive disorders was identified, and the impact of these factors on the quality of life of patients was assessed.
The study was based on a comprehensive examination of 147 patients aged 18 to 60 who were diagnosed with anxiety-depressive disorders with symptoms of eating disorders.
Patients were divided into two groups. The selection of patients to the main group and the comparison group was random, which ensured randomization. The treatment of patients in the main group was carried out according to a comprehensive, phased, differentiated treatment and rehabilitation program with a psychoeducational component, while the participants in the comparison group received a standard treatment algorithm in accordance with clinical guidelines.
The patients enrolled in the study underwent an initial examination before the start of treatment and a control examination 6 months after the start of therapy.
During the study, clinical and anamnestic, clinical and psychopathological, pathological and psychodiagnostic studies were conducted, supplemented by psychometric scales and statistical research methods.
A scientifically substantiated and implemented comprehensive step-by-step differentiated therapeutic and rehabilitation program with a psychoeducational component has been developed and introduced into practical healthcare. It includes an elaborate scheme of psychotherapeutic and psychopharmacological interventions for patients with anxiety-depressive disorders with eating disorders. An assessment of its effectiveness has been conducted, and the therapy results between the patients of the main group and the comparison group have been compared.
This program included four stages:
1. Diagnostic stage
2. Therapeutic stage:
• psycho-educational work;
• pharmacotherapy;
• cognitive-behavioral therapy (with an emphasis on cognitive models of eating behavior)
3. The catamnestic stage
The practical significance is to study the clinical and phenomenological features of eating disorders in patients with anxiety-depressive disorders and to develop, scientifically substantiate and implement in the practical activities of the health care system (for psychiatrists, psychotherapists, psychologists, psychiatrists, psychologists, and psychiatric interns) of a comprehensive, phased, differentiated treatment and rehabilitation program with a psychoeducational component, which includes a detailed scheme of psychotherapeutic and psychopharmacological intervention for patients with anxiety-depressive disorders with comorbid eating disorders. The cognitive models of the leading psychopathological syndromes were formed in order to identify psychotherapeutic targets and optimize therapeutic effects. Predictors of risks and chances of re-hospitalization during the next year after the intervention were identified. The developed prognostic multivariate logistic regression model of factors can be recommended for use in practical medicine by psychiatrists, psychotherapists, medical psychologists and general practitioners(family physicians).
