Pryputen A. Development and evaluation of the effectiveness of a physical therapy program for people of working age with neck pain (Cervicalgia).

The dissertation is about the development and evaluation of the effectiveness of a Physical Therapy Program of Working-Age Individuals with Cervicalgia of the Cervical Spine. The first chapter, presents the results of the review and analysis of domestic and foreign literature on the issue of physical therapy of working-age people with cervicalgia of the cervical spine. It is established that this syndrome is very common and acquires an epidemiological character among patients with pathology which causes functional impairments in the cervical spine, painful sensations, muscle spasms, limitation in the professional and in everyday activity. Most authors in their programs cite traditional approaches with the use of physical exercises, alternate combination of usage of cold and hot compresses, physiotherapy, wearing the Schantz collar, dry and underwater traction, therapeutic massage and acupuncture, however they don’t consider existing disorders on the physical, psychoemotional, social level pursuant to the domains of the ICF and taking into account the factors that affect the quality of life of working-age people with cervicalgia. Scientific works with a justified application of detected disorders on the level of the structure, function, activity, participation and environmental factors in working-age people with cervicalgia and theoretical substantiation application of methods and means were not found in the researched sources. The second chapter describes clinical and instrumental methods of examination to identify functional limitations in the cervical spine: goniometry was used, palpation was used to assess muscle lubrication, muscle weakness - manual muscle testing, pain assessment - visual analogue pain scale (VAS), McGill questionnaire (MPQ), MOS SF-36 questionnaire, Canadian occupational performance measure (COPM). The third chapter describes the characteristic of the functional state of the musculoskeletal system, physical state and quality of life of working-age people with cervicalgia at the examination stage before the physical therapy program. It was found that most patients had a constant static load on the collar zone and stress. The main complaint of patients in both groups was severe pain. Patients with cervicalgia syndrome had significantly reduced physical and psychological health indicators according to the MOS SF 36 questionnaire. The fourth chapter is dedicated to the construction technologies of the author’s program of physical therapy of working-age people with cervicalgia of the cervical spine. Two algorithms were developed which were directed on quality improvement of the rehabilitation diagnosis of patients and the sequence of conducting the author’s physical therapy program of working-age individuals with cervicalgia of the cervical spine. The fifth chapter presents the results of examinations of functional indicators in the cervical spine (flexion, extension, laterofiexion to the right and left side) painfulpainful sensations at rest, during active movements and during palpation, muscle tone state, evaluation for MOS SF 36, Canadian occupational performance measure (COPM), which were conducted both before the author’s program and on the 21st day after. The results of the study confirmed the effectiveness of the used comprehensive physical therapy programme, which consists in reducing the severity of pain syndrome, improving functional parameters in the cervical spine and quality of life. The results of the impact of a comprehensive physical therapy programme on the condition of patients with cervicalgia syndrome are presented. In the sixth chapter, an analysis of the obtained results was conducted and their comparison with the conclusions and results of other researches, data from scientific and methodological literature.
