The aim of this work is to scientifically substantiate and develop an algorithm for the use of physical therapy measures for women with obesity and first-degree osteoarthritis of the knee joints.
The study was conducted on the basis of the department of radio-induced general and endocrinological pathology of the Scientific Center of Radiation Medicine of the National Academy of Sciences of Ukraine, the sports and health center "Dog & Grand CrossFit" and the laboratories of the Scientific Research Institute of the NUPESU. The contingent of subjects: women aged 35-50 with obesity and gonarthrosis of the first degree, who were divided into two groups: the control group (CG, n=34), who practiced according to the generally accepted program of the medical institution, and the main group (MG, n=34), who practiced according to the author's program. The developed program was implemented in three periods and lasted 24 weeks. At the level of structures and functions, the following means of intervention were proposed: in the basic component – therapeutic exercises and dietary nutrition; in the variable component – drug therapy, prescribed by a doctor, physical therapy, therapeutic massage, mechanotherapy, balneotherapy. In the control group, the intervention plan was standard and offered by the rehabilitation institution. Evaluation of the results of the final examination (according to the Lysholm J., Gillquist J. questionnaire, VAS scale, goniometry, EMG, biochemical blood analysis) regarding the effectiveness of the proposed FT algorithm - interventions for women with obesity and osteoarthritis of the knee joints, confirmed its undoubted superiority over traditional methods.
Scientific novelty of the dissertation work:
- for the first time, based on the analysis of disorders of the structures and functions of the musculoskeletal system and lipid metabolism, an algorithm for the use of physical therapy measures for women with obesity and osteoarthritis of the knee joints of the first degree has been scientifically substantiated and tested. The basic component of the program is aimed at correcting body weight and preventing complications caused by obesity. The differential (variable) component of the program is aimed at correcting functional disorders arising from gonarthrosis in obese patients;
- for the first time, an intervention algorithm based on a combination of the use of therapeutic exercises was proposed to improve the condition of the knee joint and health fitness, to normalize the balance of the bioimpedance composition of the body in women with obesity and osteoarthritis of the knee joints of the first degree;
- expanded theoretical ideas about the most significant factors that influence the development of osteoarthritis in women of the second mature age with obesity.
The main provisions of the dissertation research are implemented in the practical activities of the Department of radio-induced general and endocrinological pathology of the Scientific Center of Radiation Medicine of the National Academy of Sciences of Ukraine, the Rehabilitation department of the State University "Institute of Traumatology and Orthopedics of the National Academy of Sciences of Ukraine", as well as for the improvement of educational disciplines, lecture courses, practical and seminar classes for students of specialty 227 Physical therapy, occupational therapy of the National University of Ukraine on Physical Education and Sports during the teaching the course "Clinical rehabilitation management during the musculoskeletal system disorders".
