Essay

The object of the study is the process of physical therapy of patients with aphasia caused by acute cerebrovascular accident. The aim of the study: to scientifically substantiate and develop an algorithm for the use of physical therapy measures for persons with sensorimotor aphasia due to cerebrovascular accident. The research was conducted on the basis of the neurorehabilitation department of the Municipal non-profit enterprise "Olexandrivska Clinical Hospital of the City of Kyiv". 62 patients after acute cerebrovascular accident with sensorimotor aphasia were allocated into 2 groups: intervention group-1 (32 patients) and comparison group-1 (30 patients). The difference in the physical therapy programs for the groups of study participants was the use in the intervention group of facilitation and visualization methods during the classes, which made it possible to achieve a greater degree of activity in the performance of exercises by the patients, as well as in the active involvement of the patients' caregivers in the physical therapy classes. Patients were examined 3 times: the first time – before the start of physical therapy measures, the second time – 2 weeks after the start of the intervention, the third time – 4 weeks after the start of the intervention. Barthel index, Rivermead mobility index and Rankin scale were used to evaluate the effectiveness of the intervention. In addition to patients, their informal caregivers were included in the study, for whom quality of life was assessed using a short Word Health Organization questionnaire: the first time – before the start of the intervention, the second time – before the patient's discharge from the hospital, the third time – a month after discharge. To assess the dynamics of the quality of life of caregivers of patients, they were also divided into 2 groups, according to the distribution of patients: intervention group-2 and comparison group-2. The practical significance of the obtained results was that the implementation of the developed algorithm of physical therapy measures contributed to increasing the efficiency of inpatient rehabilitation of stroke survivors, in particular, increasing their mobility, activity and independence, as well as increasing the quality of life of informal caregivers of the injured. The scientific novelty of the dissertation is that: for the first time, an algorithm for the application of physical therapy measures for patients with sensorimotor aphasia due to an acute violation of cerebrovascular accident at the inpatient stage of rehabilitation was scientifically substantiated and developed; for the first time methodological approaches to the application of physical therapy measures in patients with sensorimotor aphasia due to cerebrovascular accident were substantiated; for the first time, data on the dynamics of the quality of life of informal caregivers of patients with sensorimotor aphasia during physical therapy were obtained; added data on the peculiarities of planning and implementation of rehabilitation intervention in patients with sensorimotor aphasia after cerebrovascular accident; added data on the impact of the disease on the quality of life of informal caregivers of patients with cerebrovascular accident and aphasia. The results of the research are implemented in the work of the neurorehabilitation department and the neurology department of the Municipal non-profit enterprise " Olexandrivska Clinical Hospital of the City of Kyiv ", the physical rehabilitation center "Feniks" (Kyiv), as well as in the educational process of the Department of Physical Therapy and Occupational Therapy of the National University of Ukraine on Physical Education and Sport.
