In the dissertation study, an actual scientific and practical problem of health care was solved - a scientific medical and social justification of the conceptual university clinic (UC) was carried out and a modern functional and organizational model of the UC was developed.
In the course of remote research, a systematic and historical analysis of national and international scientific sources was carried out regarding the organization of the activities of university clinics abroad and the priority directions of the development of UC in Ukraine. In the course of the research, it was found that the University Clinics have a thousand-year history. An academy was founded in the Ancient East (in the Persian city of Gondishapur in the 3rd century AD). It included a university, a library and a university hospital. Philosophy, medicine, theology and other sciences were planned to be taught at the academy.
In Europe, academic medical centers began to emerge during the Middle Ages and the Renaissance. Currently, foreign university clinics operate according to two models: cooperation between the university and the clinic; integration of the clinic into the university The revival of university clinics in Ukraine began at the beginning of the 21st century. There are currently ten Criminal Codes of Ukraine operating in Ukraine.
In contrast a clinical hospital, based on which the educational process is organized, in a university clinic, in addition to educational activities, scientific research should be carried out, and the obtained results should be implemented in clinics. Unfortunately, this process does not take place in all Criminal Codes of Ukraine.
In the study, based on the materials of the official strategic reporting, a comprehensive analysis of the state of health of the population of the Kyiv was carried out and identified the main trends and challenges to public health in the capital. The analysis of morbidity, prevalence and disability was carried out according to statistical data summarized on the basis of reports of health care facilities of the system of the Department of Health of the executive body of the Kyiv City in dynamics for 2014-2023.
Also, based on the materials of the official strategic reporting, the results of a sociological survey of 409 patients, 101 doctors, 407 students, expert assessments, and a SWOT- analysis, a description of the state of the organization of the university clinic of Bogomolets National Medical Univesity. Based on the analysis of the obtained results, it was concluded that the activity of the university clinic needs to be optimized.
In the study, the conceptual organizational and methodological approaches to the development of a modern functional and organizational model of the UC were scientifically substantiated, a conceptual functional and organizational model of the UC was developed, and the management scheme of the conceptual model of the UC was executed.
From the standpoint of a systemic approach, the conceptual functionalorganizational model of the UC represents an integrated academic medical center that integrates a set of: - clinical, scientific bases, and the base of the organization of medical education, which provides conditions for the trinity of clinical practice, medical education, and scientific activity.
The conceptual model of the UC is a multi-level clinical base for the provision of primary, specialized, palliative, rehabilitation medical care with the participation of scientific and pedagogical workers of NMU, which ensures the training of students and advanced training of medical workers according to the standards of higher education, conducting scientific research, developing and introducing new medical technologies. An expert evaluation of the developed model was carried out. The conceptual model of the UC has been tested.
