Thesis is dedicated to the problems of acute cholecystitis treatment of 441 patients by means of videolaparoscopic surgery. The indications and contra-indications to this operation are specified. It is determined that the surgical treatment of the patients with complicated forms of acute cholecystitis by means of laparoscopic surgery must be built on the principles of active surgical tactics. The tactics of top priority of laparoscopic ablation of inflammation and securing of adequate derivation of bile is proposed. Adjustment of accompaning affections of duct is carried out when there is no acute conditions. For pathology diagnostics of common bile tract the preference is given to intraoperative laparoscopic cholangiography. Laparoscopic cholecystectomy is one of the methods of acute cholecistytis treatment. The main results are applied in practical medicine and 10 times brought down the post-operative mortality, 7,3 times of after-effect of an illness and 5,3 times of being in a hospital in compariso n with traditional method.
