The work is devoted to development of new scientific approaches to pathogenesis, treatment, prevention, prognosis and rehabilitation of the patients with chronic salpingo-oophoritis with pain syndrome with the account of the state of vegetative nervous system and immune resistance of the organism. To achieve the aim 145 women, of them 125 with chronic salpingo-oophoritis with pain syndrome and 20 healthy women (controls) were studied. The diagnosis of chronic salpingo-oophoritis with pain syndrome was made on the basis of clinical laboratory and instrumental (ultrasoud, EEG) findings as well as results of immune assays. The investigation of the patients with chronic non-specific salpingo-oophoritis with pain syndrome demonstrated disturbances of protective functions of non-specific immune reactivity of the organism which provides development of autoallergization of the organism accounting for prolonged course of the disease. Dysfunction of unspecific system of the brain causing the development of vege tative dystonia syndrome and aggravating pain syndrome was revealed. Using the results of the performed study the author worked out a scientific concept of the pathogenesis of chronic non-specific salpingo-oophoritis with pain syndrome which is based on reduction or overstrain of protective functions of non-specific resistance characterized in the terminal phase of immune system modulation by recovery, autoallergization and autosensibilization of the organism. The obtained data allowed to optimize the therapeutic program in the patients and prevention of the relapses. Pathogenetically substantiated therapeutic tactics including non-medicamental therapy (hyperbaric oxygenation, local hypothermia, vaginal laser therapy) allowed to normalize hemopoiesis and activation of the protective functions of the organism in the system of immune resistance, which provided recovery in 92.5% of the patients.
