Metoprolol, bisoprolol and carvedilol have been detected can be recommended in the treatment of patients with ischemic and nonischemic heart failure. Carvedilol is more effective in patients with heart failure because of its a-blocker and antioxidant properties. b- adrenoblockers dose is an individual and increasing from minimal. The first (initial) dose for metoprolol is 5 mg, for bisoprolol - 1,25 mg, for carvedilol - 3,125 mg. This dose is increasing twice in 8-10 days in patients with ejection fraction of left ventricular less then 40% and in 4-5 days in patients with ejection fraction 40-50% if arterial pressure is not fooling less then 90/60 mm hg or heart rate is not less then 60 and clinical symptoms of heart failure is not worse. The investigation of systolic and diastolic function sings in 4 weeks is recommended in patients with heart failure who received b-adrenoblockers for detecting a criterions of possible effect of fouling therapy.
