The dissertation is devoted to the problems of increasing the efficacy of therapeutic measures in patients with chronic glomerulonephritis (CGN). The clinicolaboratory course of the disease has been studied and on the efficacy of pathogenetic treatment of such patients determined. The state of NO metabolism (the nitrate/nitrite level, arginine and cyclic nucleotide aminoacid) in CGN and animals with experimental glomerulonephritis has been studied to prove that some disturbances develop in the NO system of the blood and urine which depend on the clinical course of the disease. The use of nebivalol, a NO synthesis stimulator, in animals with experimental disease results in the restoration of NO metabolism. The advisability of treating CGN patients with a combined use of nebivalol in a dose of 5 mg/day and ACFI in average daily doses favorably effecting both arterial hypotensim and other symptoms of the disease (nephrotic syndrom, renal function) has been substantiated. Nebivalol included into a complexof therapeutic measures potentates the action of glucocorticoid hormones and cytostatics. At the some time the efficacy of nebivalol has been shown to decrease with ageing of patients and increasing duration of the disease. The potassium level in the urine >77 mmol/24hh and that of oxypurinol in the blood <117 mmol/l are positive prognostic signs of the efficacy of nebivalol.
