Petyunin O. An evaluation and prognosis of the results of surgical treatment of the patients with liver cirrhosis in stage of compensation

The object is liver cirrhosis (LC) in stage of compensation; the aim to improve results of surgical treatment in patients with LC in stage of compensation by individualized choice of method of surgical operation and prognosis of results of surgical treatment; the methods are clinico-laboratory, morphological, morphomethrical, ultrasound dopplerography, contrasting radiography, statistical and questionnaire; for the first time are determined quantitative descriptions of structural changes in liver in patients with cirrhosis in stage of compensation, which allowed more exactly to estimate its functional reserves; for the first time established, that condition of vessels of portal system and portal hemodynamics associated with character of morphological changes in liver in patients with LC; proved, that operation extraperitonization of right hepatic lobe with intraoperative laser irradiation improves functional condition of liver and life quality, and operation ligation of left gastric artery and vein, splenic artery causes decreasing of expressiveness of portal hypertension in patients with LC in stage of compensation; determined new risk factors of negative result of surgical operation in patients with LC in stage of compensation; results - are worked up the new indications to surgical treatment of patients with LC, method of prognosis of results of operative interference in patients with LC in stage of compensation, that improve results of surgical treatment; all this thesis has been developed by surgical departments in hospitals; the field is Medicine.
