Object - patients with II - III stage of arterial hypertension, which was complicated and not complicated with chronic heart failure.The aim - to optimise the treatment of patients with II - III stage of arterial hypertension, which was complicated and not complicated with chronic heart failure by studding the renin-angiotensin-aldosteron system functional status in case of ACE inhibitor, beta-blocker and diuretic administration, and by the definition of indications for using of aldosterone antagonists at this patients category.Methods - clinical and laboratory-instrumental - blood and urine analysis; determination of plasma and erythrocyte membrane electrolytes levels, definition of plasma renin activity, angiotensin II and aldosterone plasma levels; electrocardiography; echocardiography; 24-hours monitoring of electrocardiogram and blood pressure.New approach - categories of patients with arterial hypertension, who needs in aldosterone system status correction, were determined: patients with III stage of arterial hypertension (especially in case of diuretic usage), persons with concomitant chronic heart failure.Results - it was shown, that spironolactone administration (25 mg daily) leads to adequate correction of aldosterone system status.All this thesis has been developed by cardiological and therapeutical departments of Dniepropetrovsk clinics, Dniepropetrovsk state medical academy, Zaporigya state medical university.The field is medicine.
