The dissertation is devoted to investigation the peculiarities of heart and extracranial vessels remodelling, related hemodynamic disturbances in patients with essential arterial hypertension (EAH) and formation of their differential correction. 128 patients with EAH of second stage were examined. It was traced that heart changes in patients with EAH have multivalued character: 64,8% of patients have left ventricle hypertrophy (LVH) (32,8% of them have concentric and 32,1% excentric LVH), 10,9% have concentric remodelling and 24.2% patients have normal heart geometry. It was proved that process of left ventricle remodelling in patients with LVH accompanied with disturbances of it's systolic and diastolic function. Increasing of "pressure loading" in the night-time facilitate remodeling of head magistral arteries and decreasing of linear and volumetric blood stream rate in carotid arteries. Prolonged usage of Enalapril in patients with EAH facilitate regression of LVH, lowering of heart sphericity, betterment of systolic and diastolic function, decreasing of complex intima-media gauge and increasing of carotids lumen with further betterment of cerebral blood flow. Influence of Diltiazem on progresses of heart and vessel remodelling is less denominated
