Dissertation is devoted to investigation of "expansion" phenomenon of left ventricle infarction zone in acute myocardial infarction, dynamics of it's creation, impact on left ventricle systolic and dia-stolic function, effectiveness of medical treatment by A II system inhibitors and optimization of treatment of these patients. 145 patients were investigated. Expediency of early, since the first day of acute myocardial infarction, use of enalapril and losartan in complex therapy in patients with left ventricle early re-modeling, that prevents acute left ventricle insufficiency, redevelopment of expansion and improvement of left ventricle systolic function was shown. It was proved, that these effects are more expressed in losar-tan. Later enalapril use (since the 4th day) in comparison to early use has less effect and does not cause redevelopment of expansion
