Object of the study: ischemic heart disease. Purpose of the study: The aim of the work was to study the role of endothelial dysfunction in the progression of stable and unstable angina, to evaluate diagnostic and prognostic value of the endothelial dysfunction as well as to assess its role in the realisation of medicinal therapy effects. Methods of the study: With the aim of endothelial dysfunction state study, the assessment of endothelin-1, cyclic 3'-5'guanosine mono phosphate (cGMP), bradykinin, angiotensin II levels was made with the use of immune enzymatic assays, and catecholamines level - with spectrofluorimetric method. To study blood lipid spectrum, measurement of the levels of total cholesterol (TCl), triglyceride (TG), high-density-lipoprotein cholesterol (HDL Cl), low-density-lipoprotein cholesterol (LDL Cl), very low-density-lipoprotein cholesterol (VLDL Cl) with enzymatic method with typification of hyperlipoidemia (HLP) according to Fridrexon. Practical significance of the obtained results: Practical significance of the work consists in the establishment of laboratory criteria for evaluation of endothelial function in patients with stable and unstable angina at baseline conditions and during medicinal correction with ACEI, ARAII and their combination. Novelty of the obtained results: Scientific novelty consists in the reveal of the relationship between activities of vasoconstrictors (endothelin-1, angiotensin II) and vasodilators (cGMP, EFR, bradykinin) and the form and severity of angina pectoris clinical course, its correlation with clinical and haemodynamics parameters as well as with the indices of sympathoadrenal system activity. Introduction: The results of the work are introduced in the pedagogical process of the Department of Hospital Therapy and Clinical Pharmacology of Kharkov State Medical University of Ministry of Health of Ukraine, in the practical work of the clinic of the Institute of Therapy of AMS of Ukraine (Kharkov), medical institutions of Kharkov - 27-th clinical hospital, 28-th clinical hospital, 20-th students' hospital, which is verified with certification of introduction. Sphere of application: in medicine of cardiology.
