Complex clinical, computer tomographic (CT), ultrasound, angiographic investigation in 300 patients with aneurysmatic subarachnoid haemorrhages till day 30 was performed. Endpoint evaluations includes Barthel index. Correlation analysis to determine course prognostic factors was used also. The pathology course was influenced by clinico-anatomical form of the haemorrhage, functional outcome depended from symptomatic secondary vasospasm (the one was frequently observed in nonoperated patients). Late admission was due to atypical haemorrhage course. According to CT data the following factors influence on the pathology course and, finally - functional outcome: presence of massive, dispensable subarachnoid blood clots, great volume of parenchimatous and massive ventricular clot. Quantitative characteristics of subarachnoid blood clot were strictly correlated with secondary ischaemia development. Unfavourable outcome risk factors were developed. Theoretical picture and new approaches for the persistent problem resolving were revealed. The correct paths for complex neurological affections evaluation in the group of patients were settled.
