Object of study: blunt or penetrating trauma which caused multiple and combined injuries of abdominal organs including spleen. Purpose of study: improvement of patients with splenic trauma and multiple and combined abdominal injuries management results. Methods of study and equipment: clinical, biochemical, immunological, morphological and statistical studies. Additional techniques: rontgenological, electrocardiographic, ultrasonographic studies and computed tomography. Theoretical and practical results: the possibility of splenic preservation in the substantial part of patients with multiple and combined abdominal injuries had been demonstrated. The improved surgical tactics regarding the peculiarities of patients group had been suggested. New methods of splenic-saving operations and intraoperative haemotransfusion were introduced, which allowed to increase the haemostasis efficacy and to widen the indications to the reconstructive operations on the injured spleen. Novelty: the new individualized tactic approach to the performance of organ-preserving operations on the spleen was suggested, the three-grade classification of organ-preserving operations on the spleen was worked out regarding the necessary haemostatic reliability and the degree of splenic injury. Degree of application: the basic principles and conclusions of dissertation had been applicated to the clinical practice of departments of traumatic shock, resuscitation and intensive care of Institute of General and Urgent Surgery of Ministry of Medical Sciences of Ukraine and surgical departments of Kharkov Municipal Clinical Hospital of Emergency Medical Care named after prof. A.I.Meshchaninov. Field of application: medicine, surgery.
