Essay

The dissertation deals with the improvement of treatment of patients with angina pectoris with concomitant chronic obstructive bronchitis. It is demonstrated that ob-structive syndrome of the patients caused by increased resistance both small and large airways. The obstruction of large airway correlates with degree of nitric oxide synthesis reduction. L-arginine corrects the bronchial obstruction mainly due to de-creased of resistance of large airways, reduced endothelial dysfunction degree and requirement of nitroglycerin consumption, improved tolerance to physical load. The inhaled nitroglycerin is more effective than sublingual one because the inhaled medicine as against latter decreased the bronchial obstruction due to reduction of upper respirotary tract resistance and improved the patients' functional state. The combined (orally and inhaled) administration of isosorbide dinitrate decreases bron-chial obstruction, improves disease course, improves quality of life. Nebivolol causes moderate improvement respirotary function. At this background endothelial function is normalized, angina pectoris course is improved, tolerance to physical load is increased, patients' quality of life is improved.
