Object of the study:injuries and wounds of the abdomen and lumbar region complicated with retroabdominal hematomas. Purpose of the study:to improve the results of treatment of the patients with injuries of the abdomen and lumbar region complicated with hematomas by optimizing the methods of diagnosis, conservative and surgical treatment. Methods of the study: the purpose of the work was achieved using general clinical, laboratory, ultrasound, x-ray methods of study and magnetic resonance imaging. Practical significance of the obtained results:introduction into the practice of the developed algorithms for differential diagnosis in injuries of the abdomen and retroperitoneal space, technique for drainage of retroperitoneal hematomas in these patients allowed to reduce death rate from 66.2 to 50%. Novelty: it was proved that retroperitoneal hematomas affect the course of the acute period in traumas due to appearance of the additional source of hemorrhage and intoxication phenomena. The new multi-factor classification of retroperitoneal hematomas was developed. A technique for drainage of retroperitoneal he-matomas depending on the localization and volume was worked out (patent of Ukraine No. 52496A). Reasonability of controlled therapeutic diagnostic tactics in patients with retroperitoneal he-matomas depending on the degree of the injury severity and he-modynamics compensation was proved. Degree of introduction:the proposed measures aimed at optimizing surgical tactics in the patients with retroperitoneal hematomas were introduced into the practice of Traumatic Shock, Anesthesiology, Reanimation and Intensive Care Department of Institute for General and Urgent Surgery (Academy of Medical Science of Ukraine), Multiple Injury Department of Kharkiv City Clinical Hospital for Emergency Medical Aid.
