Essay

Object of the study: Clinical hormonal characteristics of poly-cystic ovary syndrome (PCOS) accompanied by connective tissue dysplasia syndrome (CTDS) and chronic polycystic ovaries (CPCO). Purpose of the study: To determine clinical hormonal character-istics of PCOS accompanied by CTDS, to work out the criteria for diagnosis and effective therapeutic tactics. To substantiate diagnostic criteria and therapeutic tactics (surgical and conservative) basing on clinical hormonal characteristics of PCOS accompanied by connective tissue dysplasia. Methods of the study: Somatogenetic study of the patients which includes phenotype evaluation, laboratory and instrumental studies, statistical analysis of the obtained results were used to achieve the purpose of the work. Practical significance of the obtained results: The necessity of distinguishing of a secondary form of PCOS associated with congenital CT pathology similar to true or primary PCO from clinically heterogeneous group of PCOS. The use of the developed diagnostic criteria for the described variety of PCOS limits the number of diagnostic studies to diagnostically significant which is cost-effective. Unreasonability of surgery in PCOS accompanied by CTDS was shown, the efficacy of concervative treatment was validated. Pathogenetically substantiated protocols of conservative therpay in patients with PCOS accompanied by CTDS were worked out. Novelty of the obtained results: For the first time, a new approach (somatogenetic study with syndrome analysis) was used in PCOS. Among different clinical varieties of PCOS, genetically determined syndrome with CTD signs was distinguished. Basing on somatogenetic study, complexes of clinical hormonal parameters specific for PCOS accompanied by CTDS and CPCO were determined, which provides new differential diagnostic approaches. High diagnostic significance of external phenotype for differential diagnosis of various clinical forms of PCO was established for the first time. Reasonability of somatogenetic study of PCOpatients was substantiated for the choice of therapeutic tactics (conservative or surgical). Introduction: The results of the research were introduced into the clinical practice of Kharkiv Regional Clinical Hospital, Chuguiv Central District Hospital (Kharkiv region), Glukhiv Central Hospital (Sumy region), Maternity Hospital No. 2 (Kharkiv), polyclinics No. 3 and 6 (Kharkiv) as well as teaching process of Perinatology and Gynecology Department of Kharkiv Medical Academy for Post-graduate Training. The material of the thesis was used in the informational letters "Diagnostic approaches and choice of therapeutic tactics in polycystic ovary syndrome accompanied by connective-tissue dysplasia" and "Conservative treatment of patients with polycystic ovary syndrome accompanied by connective tissue dysplasia". Sphere of application: medicine, obstetrics and gynecology.
