Essay

Object of the study: oligomenorrhea and secondary amenorrhea in adolescent girls aged 13 to 18 years Purpose of the study: improvement of nonhormonal methods of treating adolescent girls with oligomenorrhea and secondary amenorrhea, as well as determination of prognostic factors concerning effectivity of the methods. Methods of the study: clinical examination, instrumental, biochemical, immunofermental, radioisotopic and statistical. Practical significance of the obtained results: the list of risk factors of oligomenorrhea and secondary amenorrhea occurring in adolescent girls was extended, the factors being: the girl being brought up in the family of social risk, as well as menstrual function disorders from menarche onset. The use of proposed sets of nonhormonal treatment increases its effectivity to 85 % in oligomenorrhea and to 59,3 % in secondary amenorrhea. Determination of favourable and unfavourable factors makes it possible even during the first visit of the patient to the doctor to prescribean individually selected set of nonhormonal treatment techniques. Novelty of the obtained results: correlation of hormonal status of the organism with some risk factors and clinical manifestations of menstrual function disorders in puberty was established. Four variations of pituitary gonadotropic function in adolescents girls with oligomenorrhea and secondary amenorrhea were singled out, namely normo-, hyper-, hypo- and dysgonadotropinemia. Prolonged secondary amenorrhea occurred in 27.7 % of adolescent girls against the background of decreased blood PRL level. It was shown that hormonal disorders in adolescents with menstrual function disturbances grow more severe with an increase of menstrual age. Highly effective new hormonal treatment methods for adolescent girls with said pathology were worked out, and prognostically favourable and unfavourable factors concerning effectivity of the treatment methods were determined. Introduction: everyday practice of children's and adolescents's gynecologists and therapeutists Sphere of application: practical health care, children's gynecology and pediatrics.
