Object of the study: 109 patients with acute myocardial infarction (MI). Purpose of the study: To prevent development and optimize the treatment of cardiac failure in patients with acute MI basing on the study of pathogenetic role of cytokines and their correction with ACE inhibitors. Methods of research: Clinical instrumental: physical examination of the patients, x-ray television of the chest organs, electrocardiography at rest, Holter's ECG monitoring, quantitative echocardioscopy. Laboratory: immunoenzyme method of anti-inflammatory cytokines determining, semi-quantitative method of S-reactive protein determining, creatin-phosphokinase fraction determining. Theoretical and practical results: Additional markers of inflammation and necrosis were determined; dynamics of anti-inflammatory cytokines amount in MI patiennts was shown, new methods of assessment of MI complications development basing on the analysis of objective parameters reflecting the disturbances in cytokine balance were worked out, reasonability of administration of ACE inhibitors in patients with MI was substantiated pathogenetically. Novelty: The dynamics of inti-inflammatory cytokine isolation in patients with acute MI was determined; correlation between the levels of anti-inflammatory cytokines, S-reactive protein, creatin-phosphokinase fraction and clinical manifestations of MI and the most informative diagnostic and prognostic criteria of the course was established, new mechanisms of ACE inhibitors action were studied with consideration of correction of immune parameters of the blood of MI patients. Degree of introduction: The findings of the research were introduced into the work of Institute of Therapy (Academy of Medical Sciences of Ukraine), City Hospitals No. 27 and 8, Dergachi and Zmiev Central District Hospitals.
