Essay

. The thesis is to problems of abdominal aortic aneurysmes reconstruction operations anesthesiological management. Main factors leading to systemic and renal haemodynamic disturbances have been determined on the base of central haemodynamic, renal function and oxygen transport analysis. It has been demonstrated, that discrepancy between circulating blood volume and vessel bed capacity is the main cause of these disturbances. Based on the obtained results the systematized algorithm of infusion, transfusion and correcting therapy has been developed. This algorithm is built according to attendant pathology and surgery stage. Adoption of this algorithm in the daily practice of the department of anaesthesiology and intensive care of the Institute of surgery and transplantology made it possible to shorten the patients' stay in ICU in 1,5 day and to reduce perioperative acute renal failure rate from 3,2 to 1,6%.
