Essay

The dissertation is devoted to the problem of the improvement of diagnosis of the disorders in vegetative homeostasis in children with gastroesophageal reflux and the development of the new approch to the correction of these disorders. The complete study of the peculiarities of the vegetative homeostasis in children with gastroesophageal reflux with its chronobiological and circadian characteristics was carried out for the first time. It was revealed that the significant changes in vegetative homeostasis indicate on the vegetative dysfunction in these patients. The severity of the vegetative dysfunction correlates to the severity of the motility disorders of the upper part of gastrointestinal tract. The disorders in the indexes of statistic and spectral analysis of heart rate variability that indicate on the sympathic autonomic stimulation were noted for the first time. The disorders in circadian rythm of the autonomic nervous system functioning in children with gastroesophageal reflux were found out forthe first time too. 24-hour sympathic activation in vegetative homeostasis is the main characteristic feature of these disorders. The new method of the correction of the disorders in vegetative homeostasis with cisapride was worked out for the first time. It showed to be an effective method of the correction of autonomic disorders in children with gastroesophageal reflux. The fact of the normalisation of circadian characteristics of the vegetative homeostasis after the treatment with cisapride seems to be very important as well. Any type of side effect or complication was registred during the treatment with cisapride.
