Object of the study: Chronic colitis with constipation syndrome. Purpose of the study: To improve the diagnosis of chronic colitis and to work out optimum effective methods of correction of the revealed disorders. Methods of research: To evaluate the efficacy of complex therapy for chronic colitis with constipation syndrome general clinical methods were used, ET-1, LP-AOP, IgA, M, G and CIC were investigated. To verify the diagnosis of chronic colitis endoscopic (colonoscopy, rectoromanoscopy), x-ray (irrigoscopy, irrigography), bacteriological (feces analysis for dysbacteriosis), histological (biopsy of the mucous membrane of the large intestine) methods were used. Theoretical and practical results: The performed complex examination of the patients with chronic colitis showed that increase of the disease severity was accompanied by development of endothelial dysfunction (increased synthesis of release of endothelin 1 and reduced synthesis of nitrogen oxide), disturbances of LP-AOP processes and humoral factors of the immunity. Correlation between ET-1, NO, LP-AOP parameters and humoral immunity factors was established. Reasonability of investigation of the state of vascular endothelium as an additional criterion of evaluation of the performed therapy efficacy was proven. The obtained correlation coefficients can be used as prognostic signs of the disease development. New approaches to treatment of chronic colitis with administration of the drugs improving the function of the vascular endothelium, the state of LP-AOP and humoral immunity factors, namely foci acid and cincteral were worked out. Reasonability of the suggested complex pathogenetic therapy of chronic colitis was validated. Novelty: One of the links of chronic colitis pathogenesis was re-vealed on the basis of the performed complex study of endothe-lin-1 in the blood plasma, nitrogen oxide in the blood serum, LP-AOP products in the plasma, blood serum of the patients with chronic colitis depending on the sex, age, duration of the disease. For the first time endothelial dysfunction (increased endothelin-1 level and decreased nitrogen oxide level) was determined in patients with chronic colitis. Disturbances of LP-AOP, humoral immunity factors were revealed. Correlation between ET-1, NO, LP-AOP, hu-moral immunity factors was established. Optimum protocol of treat-ment of patients with chronic colitis with the account of the revealed disturbances was worked out. Its efficacy was evaluated. A new pathogenetic substantiation of reasonability of administration of folic acid and cincteral in patients with chronic colitis and endothelial dys-function was given. Degree of introduction: The findings of the research were in-troduced into the clinical practice of gastroenterology depart-ments of Institute of Therapy (Academy of medical sciences of Ukraine), City clinical hospital No. 2, Regional clinical hospital, Regional students' hospital (Kharkiv), therapy departments of central district hospitals of Poltava, Volchansk, Novaya Vodo-laga as well as training process of Kharkiv Medical Academy for Post-graduate training, Donetsk State Medical University named after M. Gorky, Lugansk State Medical University, Ukrainian Medical dental Academy (Poltava). Sphere of application: medicine, internal medicine.
