Kamenev V. Treatment and preventive maintenance of early syphilis in a view of clinical, epidemiological, and pathological features of the disease

The dissertation is devoted to development and substantiation of a new effective method of treatment of the patients with syphilis and improvement of system of preventive measures. 16,226 patients with syphilis, registered in the Poltava oblast in 1990-2001 were analyzed, and the epidemiological and clinical features (including pathomorphosis) are investigated. An interaction of Treponema pallidum with tissues at an ultra structural level was shown. In connecting tissues of skin in patients with secondary syphilis activated lymphocytes and plasma cells were revealed. There were some Treponemas among fibers of a connecting tissues and inside macrophages in a condition of uncompleted phagocitosis. At early syphilis the severe damage of microvessels is observed. The developed treatment method was used on 537 patients with active early syphilis. The nearest and remote results of treatment by introduction 2.4 mln. EU. Of benzathinbenzylpenicillin once a week (№ 2-4) simultaneously with doxycycline 0.1 g twice a day, 10-20 days (depending on term of infection) testify to high efficiency of the given method of treatment. The syphilis prophylaxis system, developed and entered into practice on the level of oblast, is characterized by coordination of efforts of public health services and other interested departments in the shape of special programs and coordination councils under the auspice of oblast state administration.
