Essay

Dissertation is devoted to determining of the regularities of the heart structure and function remodelling, peculiarities of the coronary heart disease (CHD) follow-up in Chernobyl disaster clean-up workers (CDW) based on the data long-term observation and to optimising of the diagnostic and treatment. 900 CDW included 141 CHD patients, 200 healthy patients and 200 non add irradiated males were investigated. Clinical and electrocardiography changes, heart structure and function remodelling and exercise tolerance regularities in CDW with CHD who suffered ionising irradiation in small doses were discovered. It's shown that after nuclear catastrophe increased realizations some traditional CHD risk factors and particularly its associations are possible. Influences of negative memory about Chernobyl disaster for CHD follow-up, frequency and duration of myocardial ischemia events are determined. New approaches of angina pectoris diagnostic are substantiated and it's estimation standard have been elaborated.Measurements of treatment optimisation and estimations have been processed. Using of Angiotenzine-converting enzyme inhibitors and metabolic active therapy is substantiated. Algorithm of treatment of stable angina pectoris in CDW has been elaborated.
