Rhomanenko L. Diagnostics, prevention and treatment of ischemic disorders in cerebrum at intracerebral aneurysm.

The thesis is dedicated to problem of improvement of diagnostics, prevention and treatment of ischemic disorders at ruptures of arterial aneurysm of cerebrum. On the basis of complex analysis of clinical-neurologic and laboratory examination of 180 neurosurgical patients, which have been operated in the clinic, in combination with carrying out of neurosurgical diagnostic complex, as well as on the basis of studying of progress peculiarities of perioperational period after surgical operation with arterial aneurysm clipping, it determined risk factors of complications occurring (cerebral vasospasm, ischemic disorders).It defines main prognostically adverse factors of occurring and development of cerebral vasospasm, ischemic disorders of cerebral blood circulation within after-operation period, lethal outcomes and occurring of neurologic defects with further invalidation of patients. System analysis and complex evaluation of phase nature of changes of coefficients of hemocoagulation, water-ionic exchange,acid-base condition of patients at complicated progress of perioperational period after rupture of intracerebral aneurysm, made possible the differential application of hypervolemic hemodilution and controlled hypertension (HHH- therapy) for treatment of patients.
