Object of the study: pathogenesis and methods of correction of infertility of immunological origin. Purpose of the study: to determine the characteristics of the pathogenesis of immunological infertility and to improve the efficacy of the existing methods treatment using implantation of cryopreserved human fetal thymus tissue. Methods of the study: general clinical examination, functional diagnosis tests (test for contact of sperm and cervical mucus, basal temperature measurement), instrumental studies (hysterosalpingography and ultrasound study), determining hormonal profile (estradiol, prolactin, FSH on day 7, LH on day 14, progesterone on day 21 of the menstrual cycle) - immunoenzyme technique, study of the sperm, immunological methods: determining total leucocyte amount, percentage of lymphocytes, T-lymphocytes, T-helpers, T-suppressors, B-lymphocytes, NK-cells, T-helper / T-suppressor ratio, lymphocytotoxic test, determining hemolysins in the blood serum, ACAT titer in the cervical mucus, circulating immune complexes. Practical significance: the study allowed to suggest, substantiate and use a new effective protocol of treatment for immune fertility. The findings of the research can be used in treatment of immune fertility in women with the purpose to increase the birth rate. The use of combination of tissue therapie administration of individually selected plasma substitutes gives optimum results in treatment for immune infertility. Novelty of the obtained results: for the first time, a systemic analysis of etiological factors of immune fertility was presented, an algorithm of its diagnosis was worked out. Comparative analysis of treatment efficacy (immunomodulators, implantation of fetal thymus tissue, plasma substitutes and plasma substitutes in combination of tissue therapie with implantation of cryopreserved human fetal thymus tissue) was done for cases of immune infertility. For the first time, the efficacy of correction of immune disorders in patients with immune infertility using heterotopic implantation of fetal human thymus tissue was shown. It was shown that in patients with immune infertility the tissue therapie administration of individually selected plasma substitutes normalized the parameters of cellular and humoral immunity. Introduction: the developed methods of correction were introduced to the work of maternity hospitals No. 1, 5 (Kharkiv), city hospital No. 3, Kharkiv Regional Center for Familiy Planning and Human Reproduction, Center for Reproduction and Human Genetics "Implant". The theoretical ideas and practical recommendations of the research are used in the teaching process of obstetrics and gynecology department No. 1 of Kharkiv State Medical University, obstetrics and gynecology department No. 1 of Kharkiv Medical Academy for Post-gradate Training.
